The FAMIA Application process is now open
The FAMIA designation is designed to recognize AMIA members who:
- Apply informatics skills and knowledge within their professional setting
- Demonstrate professional achievement and leadership through the practice of informatics in their professional setting
- Provide sustained commitment to the betterment of AMIA through engagement in various AMIA activities
Application Steps
Step 1

Note: The Long-Term Experience Pathway is no longer offered. Applicants should review the current eligibility criteria to ensure they meet all requirements before proceeding.
Please be sure to read all steps prior to applying.
Complete the application by July 15, 2026.
Step 2
Purchase the $200 non-refundable application fee in the AMIA Online Store
- Confirm that you meet all eligibility criteria before completing your purchase
- Save your receipt — you will need the receipt number for your application
Step 3
Prepare and collect the following documents:
- Receipt number from your application fee purchase
- Copy of diploma (Advanced Health Degree)
- Copy of certificate(s) as applicable
- Copy of CV or resume
- Name of your AMIA peer recommender
- The recommender must be an full AMIA member in good standing (Digital Members are not eligible)
Step 4
- Log in to your AMIA account to begin your application
- Use Save to return to your application later
- Select Save and Finalize once your application is complete
Step 5
A recommendation from an AMIA Full Member in good standing is required.
After you submit your application, your recommender will automatically receive an email with instructions to complete their recommendation. They will be asked to:
- Answer four yes-or-no questions
- Provide a brief paragraph (maximum 500 words) describing why you are qualified for FAMIA
Action required:
- Notify your recommender in advance that the request is coming
- Confirm they have received the automated email (and ask them to check spam/junk folders if needed)
- Follow up with them to ensure their recommendation is submitted on time
All recommendations must be received by the application deadline.
Step 6
Application Review

Applications are reviewed by AMIA staff and at least two FAMIA members for alignment with eligibility requirements

Applications that do not receive unanimous approval are reviewed by the FAMIA Executive Committee for final determination
Step 7
Applicants will be notified once the review process is complete
Future Commitments
Individuals awarded FAMIA are expected to maintain the following:
- Certification Maintenance: Maintain any certification(s) used to qualify, as applicable
- AMIA Full Membership: Maintain continuous Full Membership (Digital Members are not eligible to hold FAMIA status)
- Engagement Requirements: Meet AMIA’s participation expectations as outlined in the Engagement Criterion
- Professional Conduct: Adhere to all AMIA policies, codes of conduct, and professional standards
