Clinical named entity recognition tools commonly map free text to Unified Medical Language System (UMLS) Concept Unique Identifiers (CUIs). For many downstream tasks, however, the clinically meaningful unit is not a single CUI but a concept set comprising related synonyms, subtypes, and associated concepts. Constructing these sets is labour-intensive, inconsistently performed, and poorly supported by existing tools.

Clinical decision support (CDS) is increasingly delivered through distributed architectures that connect electronic health record (EHR) workflows to external services and community partners. While these designs enable rapid deployment and interoperability, they also introduce new failure modes that can silently disrupt time‑sensitive care processes. Practical, real‑world descriptions of these distributed CDS malfunctions remain limited.
