AMIA urges the U.S. Department of Health and Human Services to advance a risk-based, evidence-driven framework to support responsible AI adoption in clinical care. Key barriers include limited access to representative data, insufficient infrastructure, unclear return on investment, workforce readiness gaps, and weak governance structures. We recommend transparent oversight aligned with risk, value-based reimbursement, implementation funding, inclusive decision-making, and strong privacy and accountability safeguards to ensure patient-centered, trustworthy AI integration.
