Public Biography
Dr. Sang (Sam) Pak is an Associate Professor and Director of Data Analytics and Improvement Science in the Department of Physical Therapy and Rehabilitation Science at the University of California, San Francisco. In this leadership role, he serves in various departmental quality improvement initiatives, outcomes assessment, and data governance, while also serving as an Informaticist. His career trajectory spans clinical practice, information systems analysis, and health informatics leadership, including roles as Epic Ambulatory Analyst and Lead Analyst for Meaningful Use and Regulatory Quality Programs.
Dr. Pak has helped establish data infrastructure for practice analytics, implement standardized patient-reported outcome measures (PROMIS-CAT) across outpatient rehabilitation clinics, and developed the Orthopedic Practice Dashboard (OPTICS) to support evidence-based decision-making for orthopedic residents and faculty mentors. He also directs informatics content development for the DPT program and teaches systems-based practice and quality improvement. His data-driven quality improvement work includes the successful implementation of telehealth during COVID-19 and recent initiatives addressing healthcare access disparities for patients with limited English proficiency.
Dr. Pak's research integrates health informatics with health services research, focusing on electronic health record analytics, social determinants of health, and health disparities in rehabilitation. Dr. Pak has held leadership positions as Co-Chair of the Informatics in Rehabilitation Collaborative and Vice-Chair of the Data Advisory Committee (ACAPT). He is also a certified AMIA Health Informatics Professional (AHIC) with multiple Epic EHR certifications and is completing his training as a Learning Health System Embedded Scientist (LHS E-STAR Scholar) to integrate informatics principles into the field of rehabilitation, advancing research, education, and patient care.
