The Joint Commission made medication reconciliation a National Patient Safety Goal in 2005, so why is it still such a challenge? And what happens when hospitals get it wrong? Two health system pharmacists and a former hospitalist will share real-world experiences that illustrate how accurate medication history data (or a lack thereof) makes a significant difference in patient care. AMIA informaticians and CMIOs will want to listen in and contribute to this important discussion around medication management.

Sara M. Panella, PharmD, CPh, BCPS is currently the Manager of Pharmacy Ambulatory Care, Population Health and Transitions of Care for Baptist Health South Florida Pharmacy Enterprise, in Miami, Florida. In her role, she was instrumental in standardizing the medication history and reconciliation process at Baptist Health South Florida.

 JoLeigh A. Sutton, PharmD, BCPS is currently a Clinical Pharmacy Specialist in Pharmacy Informatics at Jackson-Madison County General Hospital in Jackson, TN. Dr. Sutton obtained her Doctor of Pharmacy from the McWhorter School of Pharmacy at Sanford University in 2007. After graduation, she completed a Pharmacy Practice Residency at the University of Alabama Hospital in Birmingham, AL. In her role as a Pharmacy clinician and informaticist, she has assisted in the policy creation and implementation of the medication reconciliation process at West Tennessee Healthcare.
