Morris F. Collen Award of Excellence - Call for Nominations

Guidelines and Instructions

  • Multiple nominators of the same nominee are asked to combine their nomination into one submission and appoint one nominator to submit the nomination and be the contact.
  • Multiple nominations may be made by one nominator.
  • Once you begin your nomination, you may save and return to finish the application.
  • Your AMIA login will not work in the application software. If you are new to the software, create an account and proceed with your nomination. If you are unsure if you have an account, use the password retrieval tool to confirm that you do not have an account.

The nomination process is now closed.

Deadline: April 1, 2024

Required Information

Information needed for the nomination form:

  • Name, title, organization, and email address of the nominee
  • A short statement (max 500 words) answering the following question:
    • What do you consider the nominee’s most important contributions to the field of biomedical informatics?
  • A short statement (max 500 words) answering the following question:
    • What are the nominee’s major contributions to informatics education; the next generation of informaticians; and to diversity, equity and inclusion?
  • A short statement (max 500 words) answering the following question:
    • Why do you think the nominee should receive the Morris F. Collen Award?

The ACMI Collen Award Selection Committee may request supplementary information from the nominator. The Collen Medal will be presented at the AMIA 2024 Annual Symposium on November 10, 2024.
